A reimbursement application is submitted when applying for reimbursement for, for example, medicines, doctor’s visits, hospital and outpatient clinic fees, dental expenses, physiotherapy, or health centre fees.
HEALT FUND MEMBERSHIP APPLICATION
Please join the fund within six months of starting your employment. The application can be delivered to the fund during opening hours, dropped into the mailbox next to the door, or sent to us by email.
PENSIONER MEMBERSHIP APPLICATION
You may apply for pension insurance after 15 years of insurance coverage. The application must be submitted within one month of receiving the pension decision.
